The Adult Placement Scheme is run by Herefordshire Social Services and finds suitably kind, experienced and able carers who offer accomodation and care in their own homes for people who need support to live full and independent lives.
Placements can be made with individuals, couples or families. Currently there are about 60 carers in the scheme situated in Ross, Leominster and many other locations throughout the county.
Adult Placement carers look after a small number of people. The homes are not 'staffed' in that residents are looked after by the individual, or family who live there. Placements are usually long term although anumber of carers offer respite and/or daycare.
Adult placement homes can offer a real alternative to those who find the thought of a larger residential home daunting and would prefer a smaller family type setting.

 

Who can apply to the scheme?

Any adult with special needs (including those with serious mental health problems) is eligible to apply to the scheme.

 

What happens then?

Once a referral is made to the scheme, staff will identify suitable carers. You can visit the placement with your family or social worker before you decide if you want to live there. Your Social worker will be closely involved in the placement and adult placement workers will also visit regularly to help resolve any problems with the placement.

 

How are placements paid for?

A social worker will make an assessment of your needs and apply for funding from either Social Services or from Housing Benefit.
