Service Description:
The Service supports people with acute mental health problems who would otherwise be at risk of an admission to hospital. It also offers, to people living in the city of Hereford , programmes of intensive home treatment and support for a period of up to six weeks. The Home Treatment service may be extended across the county following an evaluation of the existing service.
All referrals to the service are assessed by qualified mental health staff use a system of triage with the aim of offering a service to those it can effectively help and of signposting to appropriate agencies those who fall outside the referral criteria.
Referrals
The Crisis Service currently only accepts referrals via the existing crisis referral pathways (link to http://www.herefordshirementalhealth.info/help/help_index.shtml) i.e via conatct with your GP or CMHT (if you are already in contact with Mental Health Services). It does not take direct or self referral.
Referrals to the team can be made during its operational hours (above) by contacting the duty worker.
