Service Description:
Oak House is an NHS run residential unit located in a street near the centre of Hereford. It provides rehabilitation and psychosocial interventions to 10 men and women with severe, long term mental health problems.
24 hour care is provided by a multidisciplinary mental health team made up of nurses, a psychiatrist and a psychologist.
The team uses a variety of medical and other interventions including Behaviour Family Therapy and Cognitive Behaviour Therapy.
It also provides continuing support to Service Users after they leave Oak House.

A Relative Support Group for relatives of Oak House residents meets once a month at 27a St Owen St, Hereford.

Referrals:
Oak House is a secondary mental health service and as such only considers referrals of people who are already in touch with Mental Health Services.
Those who are referred are screened initially for suitability followed by an assessment period of 3 months, which if successful leads to a placement offer.
