Service Description:
This service takes the form of a single Community Mental Health
Nurse available 3 days a week. She assesses anybody seen in casualty
or admitted to a medical ward following an episode of self harm.
The assessment will include the persons current mental state, the
circumstances of the recent episode of self harm, and history of
past suicidal attempts. She will also assess the level of risk that
the person is at of hurting themselves seriously or killing themselves.
She may then offer short term crisis intervention work herself in
the form of counselling usually open a weekly basis or refer them
on to a Community Mental Health Team.
The service also acts as a resource to service users and their families,
and to professionals and others who try to support them, by providing
advice, information and support in relation to self harm.
Referrals:
The Deliberate Self Harm Service takes referrals to the above number
from Medical Wards, A&E, CMHTs, doctors on call, and any other
agency that is concerned about somebody's self harm.
Referrals from medical wards are seen on the same day they are declared
fit for discharge.
People who self harm and present to A&E are assessed by the
duty doctor who may refer on to the deliberate self harm nurse or
to a CMHT if already involved.
