DMHOP is the combined Health and Social Service for people with
mental health problems who are aged 65 and over. It also has a responsibility
for people under the age of 65 with dementia illness It has in patient
beds on Cantilupe Ward for people with dementia, and on Mortimer
Ward for older people with other mental health problems. The Community
Service consists of:
2 Community Mental Health Teams
Day Hospital In Hereford City (5 days a week)
Day hospitals in Leominster and Bromyard (I day a week)
Respite Care Unit
It also provides Day Care in
partnership with several voluntary agencies at various sites around
the county.
Community Mental Health Teams (CMHTs)
DMHOP has two CMHTs, one for the north and one for the south of
the county. Both are based at the Stonebow Unit with the north team
having a further base at Leominster. The Social Worker members of
the Team are based at Moor House, Widemarsh Common, Hereford pending
a move to a joint base. The CMHTs acts as a single point of referral
for people 65 and over, seeking specialist help for a mental health
problem. They provide assessment, support and treatment in the community
and work in partnership with a range of service providers from the
voluntary and independent sector.
The two CMHTs are multi disciplinary with staff from both health
and social services and comprise (alphabetically) :
Referrals
The CMHTs prefer to take referrals from other health or social care
agencies, but are increasingly prepared to see people who refer
themselves or are referred by relatives.

Area covered:
The North team takes referrals from the GP practices in Kington,
Staunton, Hay, Leintwardine, Kingsland, Leominster, Weobley
and Bromyard. It also covers the Hereford practices at King
St, Wargrave House, Cantilupe Surgery and Sarum House. The team
may (by negotiation) also take referrals from people living
in Herefordshire but registered with a GP practice in a neighbouring
county.

The South team takes referrals from GP practices in Ross, Fownhope,
Much Birch, Kingstone, Peterchurch, Ewyas Harold, Ledbury. It
also covers the Hereford practices at Quay House, Greyfriars,
Belmont and Moorfield House.
The team may (by negotiation) also take referrals from people
living in Herefordshire but registered with a GP practice in
a neighbouring county.

Service Description:
Cantilupe is a mixed sex ward that has separate sleeping, washing
and toilet facilities for people over 65 suffering with dementia
type illnesses, cognitive impairment or confusion. The ward has
four single rooms and two rooms with five beds in each, making a
total of 14 beds.

Mortimer Ward

Mortimer is an acute psychiatric admission ward at
the Stonebow Unit providing in-patient care to people of all ages,
including the over 65s who are suffering with functional illnesses
(e.g.anxiety, depression).

Service Description:
The Nan Bellville Day Hospital offers multi disciplinary assessment,
therapy and recreational programmes for people with dementia type
illnesses. Staff also provide advice and support to carers.

Service Description:
The unit offers short term admissions of up to 2 weeks to people
with dementia type illnesses, in order to provide regular respite
to their carers.
It has ten places and is staffed by Mental Health Nurses supported
by untrained staff.
